The "Going Away" form has been created so that residents can notify the Walpole Police Department of an upcoming vacation. With this form, our officers can be aware that your home will be unoccupied for an extended period of time. We can then swing by and check to see that your house is secure while you are away.

There are two ways you can submit your form:
Click on the "Internet Version" below, fill out the form on-line, and submit it via the internet.
Click on the "Printed Version" below, print out the form, fill it out and send it in via ground mail or hand deliver it to the police station.
Both forms are exactly the same, and are quick and easy to fill out and submit; but please remember to fill out the form in it's entirety.
